Frequently Asked Questions
FMEA, or Failure Mode and Effects Analysis, is a team method for finding what could go wrong in a construction process, scoring each risk, and fixing the highest priorities first.
RPN means Risk Priority Number. You get it by multiplying the severity, occurrence and detection scores. A higher number means the risk should be addressed sooner.
A risk register usually scores likelihood and impact. FMEA adds a third score for detection, which shows how easily a problem would be caught before it causes harm.
Run it when you design or redesign a process, before high-risk tasks such as lifts, and when you introduce offsite or new methods. Review it whenever the process changes.
Lean Touch Solutions provides LCI-registered FMEA training and facilitation for contractors across Ireland, the UK and Europe.
In short:
FMEA finds problems before they cost you. Pick one process, gather the crew, list what could fail, score severity, occurrence and detection, and act on the highest RPNs. Then score again to prove the fix worked.
